Description
O cancro do Pulmão é a causa mais comum de
morte devido a cancro. A RT desempenha um
papel importante no tratamento do NSCLC. No
planeamento da RT, a CT é normalmente utilizada,
porém nem sempre fornece uma definição precisa
do volume alvo. O objetivo deste estudo é entender
qual das técnicas, 4DCT ou PET/CT, melhor define o
volume alvo e se aumenta o controle local da doença.
Este estudo consiste numa revisão sistemática da
literatura, onde foram utilizados artigos científicos
de 2006 a 2018, artigos estes encontrados durante
pesquisa realizada em bases de dados, onde foram
utilizadas as palavras-chave, os critérios de inclusão
e exclusão. Na pesquisa verificou-se que a PET/
CT e 4DCT influenciam a definição do volume alvo
de diferentes maneiras. A PET/CT fornece uma
caraterização biológica do tumor, que vai depender
do radiofármaco utilizado. Contudo, a PET/CT não
identifica o movimento que a lesão tumoral sofre
devido a movimentos respiratórios. A 4DCT fornece
informações sobre as alterações que o tumor sofre
durante o ciclo respiratório. Além disso ė possível na
PET/CT incluir um gated respiratório, permitindo uma
aquisição 4D-PET/CT. Este estudo demonstrou que
tanto a PET/CT quanto a 4DCT são importantes no
planeamento da RT, pois influenciam a definição do
volume alvo. A PET/CT permite avaliar a necessidade
de aumentar a dose e identificar os limites do GTV,
a 4DCT permite avaliar os movimentos da lesão.
Deste modo surge a necessidade unir estas técnicas,
4D-PET/CT, permitindo ter uma maior informação
para a definição do volume alvo.
ABSTRACT: Lung cancer is the most common cause of death from cancer. Radiotherapy plays an important role in the curative treatment of NSCLC, with CT being the basis for radiotherapy planning, but it does not always allow accurate target volume definition. The purpose of this study is to understand which of the techniques, 4DCT or PET/CT, more benefit the definition of target volume and which is associated with greater local control of the disease. This work consists in a systematic literature review. It was used scientific articles from 2006 to 2018, articles found during a research carried out in databases, where the keywords, inclusion and exclusion criteria were used. In the research it was found that PET/CT and 4DCT influence the definition of target volume in different ways. PET/CT provides a biological characterization of the tumor, which will depend on the radiopharmaceutical used. However, PET/ CT doesn’t identify the movement that the tumor lesion suffers due to respiratory movements. 4DCT provides information about the changes that the tumor undergoes during the respiratory cycle. In addition, it is possible in the PET/CT to include a respiratory gated, allowing a 4D-PET/CT acquisition. This study demonstrated that both PET/CT and 4DCT are important in RT planning because they influence the target volume definition. PET/CT allows to evaluate the need to increase the dose and to identify the limits of the GTV, 4DCT allows to evaluate the movements of the lesion. Therefore, it is necessary to combine these techniques, 4D-PET/ CT, allowing to have more information for the definition of the target volume.
