Description
Ao nível do Canto, muito se tem interrogado acerca da predisposição de um cantor
para o desenvolvimento de distúrbios ao nível da Articulação Temporomandibular
(ATM), dada a sua exposição à elevada mobilidade desta zona músculo-esquelética.
Efetivamente, a Disfunção Temporomandibular (DTM) é uma das desordens mais
complexas do organismo capaz de desencadear alterações nos movimentos
mandibulares que provocam prejuízos tanta na articulação da fala como na qualidade
da voz.
No sentido de verificar a relação entre ATM e produção vocal, 5 estudantes de
Canto, do sexo feminino, entre os 17 e os 27 anos de idade, a frequentar instituições
do ensino oficial e diagnosticadas clinicamente com DTM, participaram neste estudo,
sendo submetidas à aplicação de duas ferramentas indissociáveis ao diagnóstico
desta patologia, os questionários Índice Anamnésico de Fonseca (IAF) e Critérios de
Diagnóstico para Pesquisa de Disfunções Temporomandibulares (CDP/DTM). Como
complemento desta pesquisa, participaram ainda 8 docentes de Canto e 8
profissionais de saúde a fim de se compreender não só a importância desta
articulação e o impacto que um distúrbio na mesma pode inferir no processo de
ensino e aprendizagem, mas também as possíveis estratégias a implementar para a
sua prevenção ou aquando do percurso de tratamento. Verificou-se que indivíduos com grau moderado de dor, principal sintomatologia
provocada pela DTM, apresentam suscetibilidade a desequilíbrios das estruturas
funcionais mastigatórias, articulatórias e ressoadoras, emergindo na diminuição da
qualidade vocal, dada a influência resultante na ação fonatória. Observou-se, ainda,
uma redução da amplitude mandibular, conduzindo a um desconforto no momento de
abertura e/ou encerramento da mandíbula e confirmando, deste modo, a relevância
desta articulação no dia-a-dia de um cantor. Foi também possível concluir-se que os
profissionais devem reconhecer a importância da saúde e estabilidade da ATM,
tomando consciência das práticas pedagógicas e performativas adotadas e
percebendo se estas se direcionam no objetivo de uma abordagem da voz como um
todo, apesar da sua complexidade interdisciplinar, já que uma prática inadequada
pode comportar, em vários casos, um fator impulsionador ou perpetuador do
problema já existente. Por fim, evidenciaram-se como terapêuticas preponderantes a
fisioterapia e o acompanhamento psicológico. Se a primeira estratégia se apresenta
como um ótimo recurso para o alívio da dor e reeducação neuromuscular e
mandibular, a segunda expõe-se como um mecanismo de restabelecimento mental,
permitindo ajustes de condutas menos apropriadas e refletoras de hábitos rotineiros
considerados incorretos e prejudicais para o paciente portador de DTM.
Abstract : Regarding the voice, much has been questioned about a singer's predisposition for the development of disorders in the Temporomandibular Joint (TMJ), given his exposure to the high mobility of this musculoskeletal zone. Effectively, Temporomandibular Dysfunction (TMD) is one of the most complex disorders of the human body capable of triggering changes in mandibular movements that cause serious damages in speech as in voice quality. In order to verify the relationship between TMJ and vocal production, 5 students of Singing, female, between 17 and 27 years old, attending official teaching institutions and diagnosed clinically with TMD, participated in this study, being submitted to the application of two inseparable tools to the diagnosis of this pathology, the questionnaires Fonseca Anamnesis Index (FAI) and Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Complementing this research, 8 Singing teachers and 8 health professionals also participated in order to understand not only the importance of this joint and the impact that a disorder in it can infer in the teaching and learning process, but also the possible strategies to prevention or during the course of treatment. It was verified that individuals with a moderate degree of pain, the main symptomatology caused by TMD, present susceptibility to imbalances of the functional masticatory, articulatory and resonator structures, emerging in the decrease of vocal quality, due to the resulting influence on the phonatory action. It was also observed a reduction of the mandibular amplitude, leading to discomfort at the moment of opening and / or closure of the jaw and confirming thus the relevance of this joint in the daily life of a singer. It was also possible to conclude that professionals must recognize the importance of ATM's health and stability, becoming aware of the pedagogical and performative practices adopted, and perceiving if they are directed at the goal of a voice's approach as a whole, despite its interdisciplinary complexity, once inadequate practice may be, in many cases, a driving force or perpetuator factor of the existing problem. Finally, is has been also perceived as predominant therapies physiotherapy and psychological counseling. If the first strategy presents itself as a great resource for pain relief and neuromuscular and mandibular re-education, the second is exposed as a mental recovery mechanism, allowing for adjustments of less appropriate behaviors, reflecting routine habits considered incorrect and harmful for patients with TMD.
