Introdução
O acidente vascular cerebral é a primeira causa de
morte em Portugal, sendo também uma das principais causas de comorbilidades físicas e psicológicas,
condicionando a qualidade de vida. Existem ainda
várias lacunas no seu prognóstico. Deste modo, é
importante perceber qual dos dois tipos de acidente
vascular cerebral (isquémico ou hemorrágico) tem
melhor prognóstico.
Objetivo
O principal objetivo do estudo foi avaliar a taxa de
sobrevivência dos principais tipos de acidente vascular cerebral, e avaliar simultaneamente o risco de
mortalidade inerente a estes indivíduos.
Métodos
Estudo de coorte histórica com dois grupos: acidente
vascular cerebral hemorrágico e isquémico. Avaliou-
-se a taxa de mortalidade dos indivíduos, com uma
amostra de 1367 indivíduos que sofreram acidente
vascular cerebral entre 2010 e 2014, analisado o
perfil da amostra, fatores de risco e comorbilidades
associadas. Destes, criou-se uma subamostra de 311
indivíduos que sofreram acidente vascular cerebral,
no período de 2013-2014 aos quais foi realizada a
análise de sobrevivência e risco.
Resultados
Os indivíduos que sofreram acidente vascular cerebral isquémico apresentaram uma taxa de mortalidade mais baixa, assim como um melhor tempo de
sobrevida inicial. Verificou-se que 3 meses após o
evento, o risco de mortalidade deixou de ser influenciado pelo tipo de acidente vascular cerebral. Observou-se também que a idade foi um fator importante
que influenciou a sobrevida.
Conclusão
Concluiu-se nesta amostra que o acidente vascular
cerebral hemorrágico está associado a uma maior
mortalidade e menor sobrevida que o acidente vascular cerebral isquémico.
ABSTRACT: Introduction Stroke is the main cause of death in Portugal and it’s also one of the pathologies involved in several physical and psychological comorbidities, affecting life quality. There are still several gaps related to the prognosis of the two different types of stroke (ischemic and haemorrhagic) and is relevant to know which type has a better longevity. Objective The main objective of the present study is to evaluate the survival rate of both types of stroke, and simultaneously the mortality risk in those patients. Methods Historical cohort study with two groups: hemorrhagic stroke and ischemic stroke, in which the mortality rate of individuals was assessed, with a sample of 1367 individuals who suffered stroke between 2010 and 2015, we analyze the sample profile, risk factors and associated comorbidities. A subsample of 311 individuals who had suffered a stroke in the period 2013-2014 was study and underwent a survival and risk analysis. Results Individuals who suffered an ischemic stroke have a lower mortality rate, as well as a better initial survival time; It was found that 3 months after the event, the risk of mortality is no longer influenced by the type of stroke. It was also observed that age is an important factor that influences survival. Conclusion Haemorrhagic stroke is associated to an increased mortality and worst survival rate compared with ischemic stroke.
